Enrollment Forms

You will also receive a paper copy of enrollment forms during your tour. All forms must be returned to the center 2 business days before your child's first day.

Health Care Plan & Medication Form

This form MUST be given to administration and reviewed prior to implementation. This form should be accompany any medication brought to the center. All medications must not be expired and have a label. A dose or application must be given at home before we can administer.
